Note: According to §1611.A.1-3 of the Louisiana Administrative Code, 10 hours clinical CE credit may be earned online during each renewal period for the relicensure of a dentist. Also, according to §1613.A.1-2 of the Louisiana Administrative Code, 8 hours clinical CE credit may be earned online during each renewal period for the relicensure of a dental hygienist. If done online, this course requires the successful completion of a written examination at the conclusion of the course.

In order to receive a Verification of Participation letter for CE credit, each video conference registrant must complete online a short test followed by a separate brief evaluation questionnaire via Survey Monkey. The links to the test and the evaluation questionnaire will be sent before you take the course.

Cancellation & Refund Policy:
The Louisiana Academy of Continuing Dental Education, Inc./ (d/b/a: LSU Health New Orleans Continuing Dental Education) reserves the right to cancel any course should circumstances warrant such action. In the event a course is cancelled, the Academy will refund the full tuition. If you cancel your registration for this course, the Academy will refund the tuition you paid (minus an administrative cancellation fee of 10% of the tuition you paid). We must receive your written cancellation request no later than November 23, 2018 by mail, fax at (504) 941-8403, or email at
info@lsucde.org. If LSU Health New Orleans CDE receives notification of your cancellation after November 23, 2018, this will result in forfeiture of the entire tuition.
